Public Relations Event Requests
If you would like to request participation from us for one of your events, please fill out the Public Relations Event Request Form.
Custodian of Records
Chief Matt Pagano is hereby appointed custodian of the records of the Maryland Heights Fire Protection District. The custodian is located at 2600 Schuetz Rd. and shall respond to all requests for access to or copies of a public record within the time period provided by statute except in those circumstances authorized by statute. Requesting Medical Records
To speed delivery of your medical records, Maryland Heights FPD now uses a service called ChartSwap. Follow these steps to retrieve your records:
- Go to ChartSwap.com
- Register to use the service.
- After registering, login to do a record search based on your personal information.
- ChartSwap will do a search and send you a notification within 24 hours of all records found.
- Fill out the Patient Authorization Form in order to view the list of available records.
- After viewing, select the records you want to receive.
- ChartSwap will notify you as soon as your records are in the portal, where you may retrieve them.
