The Maryland Heights Fire Protection District fulfills ambulance and EMS medical records requests through ChartSwap, a secure records service. ChartSwap provides records to law firms, insurance companies, and record retrieval services; a completed patient authorization is required for every request. Register once, then request the records you need — ChartSwap notifies you when they are ready to retrieve from its secure portal.

Requesting your own records? ChartSwap cannot fulfill requests from individual patients. Patients or their legal guardians may request their own records by submitting a written, signed request to the EMS Division at medicalofficer@mhfire.org. Include the patient's name, date of birth, date of the incident, and a copy of a photo ID; requests are processed in the order received. Step by Step

  1. 01 Go to ChartSwap.com and register your firm or company to use the service.
  2. 02 After registering, log in to do a record search based on the patient's information.
  3. 03 ChartSwap will search and notify you within 24 hours of all records found.
  4. 04 Submit the completed Patient Authorization Form to view the list of available records.
  5. 05 Select the records you want to receive.
  6. 06 ChartSwap will notify you as soon as the records are in the portal, where you may retrieve them.

Incident reports, board records, and other district records are handled by the Custodian of Records under the Missouri Sunshine Law.
